Health Initiatives

Foundation Communities' Health Initiatives department is committed to improving the health of our residents and neighbors. We provide vital health care services on-site, including access to mental health care, as well as opportunities to engage in a wide range of physical activity, nutrition, and community health programs. The volunteers and staff on our Health Coverage team enroll hard-working families and individuals in affordable health insurance plans, providing them with peace of mind for the future.

 

The Health Initiatives department consists of five program areas:

Health and Fitness and Employee Wellness (fitness classes, health fairs, vaccinations, physical activity events, and employee wellness)

Clinical Support Services (mental health and case management)

Health and Nutrition (food pantries, health and nutrition education, and community health work)

Health Coverage (assistance with enrollment in ACA Marketplace health insurance plans)

Health Quality (evaluation, partnerships, and project management)
